The KSB Health Information Management Services (HIMS) department ensures that accurate, complete, and dependable health information is available for all patient care delivered by KSB Hospital staff. It also retains the health information according to state and federal laws and makes it available to all patients.
Physicians and nurses within the KSB Hospital organization have timely electronic access to patients’ health information, including records that originate in the clinics, physician offices, emergency department, and the hospital.
When a patient has a specific need for their health information, patients are encouraged to sign up for online access to their patient portal by calling (815) 285-5424 or printing the Authorization for Release of Medical Information, completing it, and faxing to (815) 285-5871 or emailing to KSBROI@ksbhospital.com
