The KSB Patient Navigator provides coordinated care management services to patients with chronic or newly diagnosed conditions who may benefit from assistance navigating through the healthcare system.
Our skilled nursing professionals provide one-on-one services to patients to find the right health plan to suit their needs and improve their quality of life.
Patient Navigators work closely with other healthcare providers, from physicians and pharmacists, to therapists and social workers, and offer education on health conditions, assistance with affordable medications, as well as helping patients to understand insurance coverage.
